Bluetooth: btbcm: Add entry for BCM43439 UART BT 



This patch adds the device ID for the BCM4343A2 module, found e.g. in
the Infineon (Cypress) CYW43439 chip. The required firmware file is
named 'BCM4343A2.hcd'.
