ide-tape: move historical changelog to Documentation/ide/ChangeLog.ide-tape.1995-2002

Also, cleanup whitespace and update comments.

Bart:
- remove reference to drivers/block/ide.c
- move driver documentation to Documentation/ide/ide-tape.txt
