dm cache policy smq: fix "default" version to be 1.4.0

Commit bccab6a0

 ("dm cache: switch the "default" cache replacement
policy from mq to smq") should've incremented the "default" policy's
version number to 1.4.0 rather than reverting to version 1.0.0.
