mm/dynamic_pool: fill dpool with pagelist

This patch introduce another way to fill the dpool. In order to achieve
this, the user should provide pfn ranges with pre-built page table and
struct page. Then the user can call dpool_init() to create a new dpool and
fill the dpool with the above pages.

To enable dpool int this way, "dpool=on" should be added to kernel
parameters.
