x86/msr-index: Define INTEGRITY_CAPABILITIES MSR

The INTEGRITY_CAPABILITIES MSR is enumerated by bit 2 of the
CORE_CAPABILITIES MSR.

Add defines for the CORE_CAPS enumeration as well as for the integrity
MSR.
