pseries: Update SLOF firmware image

The main changes are:
- fixes in PCI bridges code;
- LUN>255 are allowed not in virtio-scsi.

  > libhvcall: drop unused KVMPPC_H_REPORT_MC_ERR and KVMPPC_H_NMI_MCE defines
